Fax Exam/Immunization Records - What's required and How to Upload/Fax after Application
The MA Board of Health requires exam/immunization records from each camper that attends camps/clinics/training sessions over school vacations.
A $50 ADMIN FEE will be charged for failure to submit these med records within 48 hours after your application submission. (the forms are to be uploaded/faxed AFTER the application submission, not before) Email email@example.com to request a fee waiver, for legitimate delays or if you have technical difficulties. We do not want your extra money, but we really do need these forms!
The exam/immunization record (provided by your doctor) must include:
1. Date/results of a physical (within 24 months of camp end date) 2. List and date of all your required immunizations 3. Significant medical conditions and findings 4. Doctor's signature
