Step 2 :Gather Exam/immunization Records before Starting the application.
These records are required for all Masspike sessions, within 48 hours of application submission. Failure to comply will result in an additional $30 fee. The forms can be FAXED or UPLOADED (pdf format only) at the end of the applicaiton. Instructions on how to submit forms (read and print if needed).
We truly don’t want your extra money; just the forms.If there is a legitimate reason why you can't comply, email us to explain the circumstances and we’d be happy to work with you.
a. Records provided/signed by doctor's office b. Exam date stated (within 18 months of clinic session end date) c. All Required shots and their dates received must all be listed d. If you have forms (that meet the criteria)in the Masspike online system, you're all set. If your exam date is no longer good, upload more recent exam/immunization records.
Step 3 :Gather Credit Card Info - Full payment is usually required with enrollment (except for Lessons) Step 4: APPLY ONLINE To start your application, you will be asked for a login email address. If it matches an email in our records, you will be asked for your password. If you forgot it, or never had one, RESET the password. Then start the applicationIMPORTANT TO READ BEFORE FILLING OUT APPLICATIONS - to avoid technical frustrations. If you have trouble,
Best to use Google Chrome or Mozilla Firefox. Internet explorer- latest version will often work, but troubles are experienced. At this point, you cannot use your phone to apply.
At the end of each application page, there is a SAVE FOR LATER button. I would suggest pressing it before leaving the Billing Page or if you leave the computer. Then if you get disconnected, you will not lose the application data. It may bring you to the beginning, but you can Press the Continue button to get back to the end. (It will take an extra minute, but trust me, it's better than losing your data)
The application is lengthy. Using the Save for Later Button will allow you to save data and come back to it later. Thank you, ahead of time, for your diligence and patience. We collect all needed info and med forms during the application process. No further forms will be required after the initial collection of info.
Step 5: After application submission, go to your account to submit med forms
Step 6 : CHECK INBOX FOR EMAILSContact us if you don't receive them.
Email 1 Application submission verified - Immediately (not enrollment) (Small group lessoon applicants will receive a separate correspondence, with more info on scheduling their sessions)
Email 2: Enrollment Status/Session Info - 2-5 days later (e.g. enrolled, waitlisted)
Email 3: Credit Card notification - same time as enrollment email
Email 4: Last minute email/session info - week before session
Although you have submitted account info, you WILL NOT BE CHARGED until you are enrolled by Karyn, the director, after she checks the availability for each court. (after going through your experience info on the application) Thank you for your patience while we check the space availability and arrange group assignment for the applicant.
